Helix Health and Housing Services (Helix) is an innovative, fast-paced human service agency in Minnesota working to eliminate barriers in accessing mental health and housing services to populations throughout Minnesota that are under-served. Helix provides four (4) areas of service across the state of Minnesota: Community-Based Mental Health Services, Targeted Case Management, Community Health Worker Services, and Housing. care that is individualized to meet the needs of people served.
POSITION SUMMARY:
The Community Health Worker (CHW) plays a key role in promoting health, providing education, connecting individuals to services, and improving access to care for underserved communities. The CHW will act as a bridge between clients, healthcare providers, and community resources to address social determinants of health and improve overall client well-being.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Conduct client outreach, engagement, and follow-up to assess needs and barriers to care.
Provide culturally sensitive health education on topics such as chronic disease management, preventive care, nutrition, mental health, and substance use.
Connect clients to community services, including medical providers, housing resources, food assistance, employment support, and transportation services.
Advocate for clients by coordinating services, assisting with care plans, and supporting navigation of healthcare and social service systems.
Collaborate with multidisciplinary teams, including healthcare providers, case managers, and social workers, to support holistic client care.
Maintain accurate and timely client documentation and reporting using designated tracking systems.
Support clients in setting personal health goals and building self-management skills.
Assist in developing and delivering community health presentations or workshops as needed.
Maintain strict confidentiality and follow HIPAA regulations and agency policies.
Participate in ongoing training and professional development opportunities.
Attend required agency meetings and trainings.
Receive weekly one-on-one supervision to ensure ongoing support, accountability, and skill development.
Other duties as assigned.
KNOWLEDGE:
Understanding of basic public health principles, including health promotion and disease prevention.
Knowledge of local community resources, healthcare systems, and social service programs.
Ability to educate clients on health topics in a culturally appropriate and literacy-sensitive manner.
Cultural competency and humility when working with diverse and underserved populations.
Client advocacy skills and promoting client self-empowerment within health and community systems.
Understanding and application of confidentiality standards, including HIPAA regulations.
Strong communication skills, including active listening, verbal communication, and written documentation.
Awareness of maintaining professional boundaries with clients and colleagues.
Knowledge of self-care strategies to promote resilience and prevent burnout.
EDUCATION AND EXPERIENCE:
High school diploma or equivalent required; Associate’s or Bachelor’s degree in a health, social work, or human services field preferred.
CHW Certification required
Completion of a recognized Community Health Worker training program preferred.
Minimum of one (1) year of experience working with diverse communities or in a human services, health outreach, or community-based setting preferred.
Bilingual or multilingual skills are a plus.
PHYSICAL REQUIREMENTS:
The position requires the ability to perform various physical tasks essential to job duties. These include, but are not limited to:
Lifting: Ability to lift and carry objects weighing between 25 and 50 pounds on a regular basis.
Bending & Stooping: Frequent bending, kneeling, and stooping may be required to access materials, equipment, or perform job-related tasks.
Sitting: Prolonged periods of sitting may be necessary, depending on the nature of work.
Walking & Standing: Ability to stand and walk for extended periods, including navigating various work environments.
Reaching & Stretching: Occasional reaching overhead or in awkward positions to retrieve or handle materials.
General Mobility: Adequate physical ability to move freely within the workspace and perform assigned duties safely and efficiently.
