POSITION SUMMARY:
This position will provide individual adult mental health services to support clients with serious and persistent mental health diagnoses. Helix serves individuals in the community who have historically struggled to access services to include individuals who are experiencing homelessness/housing instability, justice-involved, and working to recover/manage substance use/ mental health symptoms. Activities may include crisis intervention, case management, client advocacy, prevention, and education. Helix's services aim to increase psychiatric stability, promote social competencies, facilitate personal and emotional stability, and maintain independent living in the community.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Provide community-based mental health case management services to qualifying adults, ages 18 and older, diagnosed with a serious and persistent mental illness.
Maintain assigned caseload and complete a minimum of 43 billable mental health encounters a month.
Complete Functional Assessments, Individual Community Support Plans, and other documentation required for Rule 79 case management.
Complete all required documentation per Minnesota statute and case notes within 48 hours of the encounter.
Engage with participants from an empowerment, strength-based perspective and promote participant self-determination and develop trusting, therapeutic relationships with participants.
Support participants in individual sessions to assist them in meeting goals around substance use, mental health, physical health, employment, transportation, housing, financial, relationships, and/or leisure time.
Refer participants to community resources, including housing and treatment programs, to assist in recovery from mental or physical illness, following through to ensure service efficacy.
Develop, monitor, evaluate and modify treatment plans according to changes in participant status from a person-centered perspective. Document participant progress.
Assist participants in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing support.
Collaborate with counselors, physicians, nurses, social service providers and other professionals to plan or coordinate treatment, drawing on social work experience and needs.
Coordinate care with others who provide services to participants. Include family members and natural support in the care of the participants, as applicable, and help assist them in understanding, dealing with, or supporting the participants.
Interview participants, review records, conduct assessments, or confer with other professionals to evaluate the mental or physical condition of participants.
Educate participants or community members about mental or physical illness, abuse, medication, or available community resources.
Increase social services knowledge and best practices by reviewing current literature, conducting social research, or attending seminars, training workshops, or classes.
Attend all agency meetings and trainings.
Other duties as assigned.
KNOWLEDGE:
Psychology - Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
Therapy and Counseling - Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.
Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
Education and Training - Knowledge of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects.
EDUCATION AND EXPERIENCE:
Bachelor's Degree in a behavioral science field required. (ex. social work, psychology, nursing, etc.)
2,000 hours spent in the behavioral health field
Valid driver's license and insurance for travel between sites and other business-related destinations
Must be able to pass a comprehensive criminal background check

PHYSICAL REQUIREMENTS:
The position requires the ability to perform various physical tasks essential to job duties. These include, but are not limited to:
Lifting: Ability to lift and carry objects weighing up to 25-50 pounds on a regular basis.
Bending & Stooping: Frequent bending, kneeling, and stooping may be required to access materials, equipment, or perform job-related tasks.
Sitting: Prolonged periods of sitting may be necessary, depending on the nature of work.
Walking & Standing: Ability to stand and walk for extended periods, including navigating various work environments.
Reaching & Stretching: Occasional reaching overhead or in awkward positions to retrieve or handle materials.
General Mobility: Adequate physical ability to move freely within the workspace and perform assigned duties safely and efficiently.
